A good rule is to describe the job in the number of steps a coworker would need to do it right. Most tasks land somewhere between eight and fifteen steps. Use plain verbs: position the ladder, tie off, run the saw, clear the guard. Record the steps as they really happen, not as the manual says they should, because the gap between the two is where people get hurt. This ordered list is the backbone of the whole job hazard analysis.
Step 4: Identify the Hazards in Each Step
Go step by step and ask what could go wrong at each one. For every step, run through the ways a worker could be hurt: struck by, caught in, fall from, contact with, overexertion, and exposure to something in the air. Do not stop at the obvious. Look for hazards that build up over time, like noise, vibration, silica dust, or a chemical vapor. Those health hazards rarely cause a dramatic incident, so they get missed, but they do real long term harm. Step 5: Control Hazards With the Hierarchy of Controls
For every hazard you named, decide how to control it. Do not jump straight to gloves and glasses. OSHA and NIOSH both use a hierarchy of controls, and it ranks controls from most protective to least. According to OSHA’s guidance on the hierarchy of controls, you select controls that emphasize elimination or substitution first, then engineering controls, then safe work practices and administrative controls, and only then personal protective equipment.
- Elimination. Remove the hazard. If you can do the work from the ground instead of at height, the fall hazard is gone.
- Substitution. Swap the hazard for something safer, like a water based product in place of a solvent.
- Engineering controls. Put a barrier between the worker and the hazard: a machine guard, local exhaust ventilation, a guardrail.
- Administrative controls. Change how people work through procedures, training, rotation, and signs.
- Personal protective equipment. The last line, not the first. PPE only protects the one worker wearing it, and only when it is worn right.

A Simple Example on a Real Task
Here is how the process looks on a task most crews know, cutting a control joint in a fresh concrete slab with a gas powered saw. Written out, a job hazard analysis for this job stays on one page.
First, break the task into steps: move the saw to the slab, fuel and start the saw, mark the cut line, make the cut, and clean up. Five plain steps, in the order they happen.
Next, name the hazard in each step. Moving the saw is a lifting and pinch hazard. Fueling is a fire and vapor hazard. Starting the saw adds noise and a moving blade. Making the cut is the big one: silica dust in the air, kickback, flying chips, and the blade itself. Cleanup brings dust and sharp debris.
Then pick a control for each, working down the hierarchy. For the silica dust, do not reach for a dust mask first. Use a saw that runs water on the blade or an attached dust collector, an engineering control that cuts the dust at the source.
